Family Physicians of Western Colorado is a division of Primary Care Partners located in the PCP Care Village in Grand Junction.
Our family physicians have been serving Western Colorado for over two generations, and we are proud of our heritage of providers with deep roots in the community. Joshua Campbell,
Doctor of Medicine
Dr. Joshua Campbell is delighted to live in Grand Junction, the place where he grew up. While working with children diagnosed with HIV/AIDS in college at the University of Colorado Boulder, he discovered a love of medicine. Dr. Campbell continued on to medical school at Loyola University in Chicago and then returned to Grand Junction to finish his family medicine training at St. Mary’s Medical Center. He has been with Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, since 2009.
Dr. Campbell is a beloved physician and maintains a loyal patient base. He has the utmost respect from his colleagues throughout the valley, and keeps a close working relationship with other medical specialties in order to provide optimal patient care. He is passionate about preventive, adult, and adolescent medicine.
When he’s not working, Dr. Campbell and his wife— who is also a practicing physician in the Grand Valley—spend their time at children’s sporting events supporting their kids and enjoying Colorado’s beauty on their mountain bikes.

Thomas Randolph Cate,
Doctor of Medicine
Dr. Randy Cate is board certified in internal medicine and has been a practicing physician since 1991. He received his medical degree from the University of Texas Southwestern Medical School in 1989 and then continued his training in internal medicine at the University of Utah. He has worked at the Salt Lake City Veteran Affairs Medical Center and at Wasatch Internal Medicine in Salt Lake City before joining Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, in 2020.
Dr. Cate has a unique approach to healthcare that focuses on the prevention of disease and health promotion, with the goal of helping his patients to live long, healthy lives. He prides himself in thorough, whole-person care, and seeks to build positive, impactful relationships with his patients.
Dr. Cate enjoys camping in Colorado, campfires, and s’mores roasted to perfection.

Britta Czapla,
Doctor of Medicine
Dr. Britta Czapla grew up in the suburbs of Pittsburgh, Pennsylvania. After obtaining her undergraduate degree from Cornell University, Dr. Czapla continued her medical training as an EMT and paramedic in Wyoming and Durango, Colorado. She then completed her medical degree at the University of Colorado.
As a dynamic, motivated physician with extensive training in the broad scope of family practice, she practices internal medicine, gynecology, emergency medicine, pediatrics, and sports medicine. Dr. Czapla has a strong interest in health promotion, disease prevention, and a commitment to the underserved.
Dr. Czapla is also extremely involved in our community. She has been a presenter of tobacco-free education to fifth graders, a volunteer for medical providers for the Colorado Mesa University cycling team, a mentor of at-risk teens, and a leader of backcountry trips for outdoor education programs. She is well-loved and devoted to the residents of the Grand Valley.
When she is not working, Dr. Czapla can be found participating in just about any outdoor activity, reading, cooking, or spending time with her family and friends.

John Flanagan,

Dr. John Flanagan joined Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, in 2009. Prior to joining FPWC, he practiced in Ft. Collins, Colorado for eight years. Dr. Flanagan grew up in Connecticut and graduated from the University of Connecticut School of Medicine in 1994. After graduating from UC, he moved to Denver, where he trained in Family Medicine.
Dr. Flanagan continued as teaching faculty at the St. Joseph Hospital Family Practice Residency, where he discovered a fondness for educating new physicians. He has also been faculty at the Northside Health Center in Fort Collins, Colorado, and St. Mary’s Family Practice Residency. He was honored with the “Teacher of the Year” Award at St. Joseph’s Hospital and St. Mary’s Medical Center. Dr. Flanagan loves that his field of practice allows him to care for the whole family and to explore family dynamics in healthcare. His special interests include pediatrics, preventive medicine, and sports medicine.

Patrick Henke,

Dr. Henke pursued his medical studies at Burrell College of Osteopathic Medicine in Las Cruces, NM, and received his Doctor of Osteopathic Medicine degree in 2020. He completed his family medicine residency at Southern Colorado Family Medicine Residency in Alamosa, CO in 2023.
Dr. Henke is passionate about treating chronic medical diseases and performing osteopathic manipulative medicine. He loves hearing his patients’ stories and works closely with them to create personalized treatment plans to optimize their health.
In his free time, Dr. Henke enjoys spending time with his wife where he says you will find them poorly attempting to trail run, mountain bike, or ski. He also likes to unwind with a good book.

Joseph Kanopsic Jr.,

Dr. Joseph Kanopsic joined Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, in 2020. He earned his undergraduate and his medical degree from the University of Arkansas and trained in family medicine at St. Mary’s Medical Center.
Dr. Kanopsic provides comprehensive treatment to men, women, and children of all ages for acute and chronic conditions as well as preventative care. He believes a person’s health should be a collaborative effort between physician and patient working together as a team. Exercise, nutrition, and lifestyle play a major part in that role, and Dr. Kanopsic believes the partnership between physician and patient can lead to longer, healthier, happier lives.
Dr. Kanopsic is a member of Mesa County Search & Rescue and enjoys backpacking, hiking, fly fishing and fly tying, skiing, and mountain biking in his free time. He loves calling western Colorado his home and playground.

Andrew Mohler,
Doctor of Medicine
Dr. Andrew Mohler was born on a United States Army base in West Germany. He grew up in Grand Junction and is proud to call western Colorado his home. He attended the University of Colorado Denver for his medical degree and received his Family Medicine training at the Good Samaritan Family Medicine program in Phoenix, Arizona. He continued his education as a clinical instructor at Good Samaritan in Phoenix where he received the “Robert A. Price” Award for Outstanding Residency Faculty Member.
Dr. Mohler values the long-term relationships that he has built with his patients. His clinical interests include management of chronic diseases, preventative medicine, and caring for families.
When Dr. Mohler is not practicing medicine, you can find him reading, hiking, running, fishing, or clearing brush on his property.

Amy Mohler,

Dr. Amy Mohler joined Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, in 2013. She is from Scottsdale, Arizona, and obtained her Doctor of Medicine from the University of Chicago. Dr. Mohler completed her Internal Medicine Residency and her Geriatric Fellowship at the Good Samaritan Regional Medical Center in Phoenix, Arizona.
Dr. Mohler is an engaging speaker and is often found educating the community, especially on Geriatrics and Palliative Care. She is an expert on topics important to senior adults such as polypharmacy, falls, dementia, and Advanced Care Planning. Dr. Mohler has also spent many years as Medical Director of a number of long-term care facilities in the area. She has dedicated her career to the care of seniors and their complex medical issues – while keeping in mind the patient’s values and morals, and how they shape their own end-of-life care.
When Dr. Mohler is not at work, she loves to spend time hiking with her family.

Monique Morisseau,

Dr. Monique Morisseau grew up in Rhode Island and became passionate about the field of medicine after her father— who had Type 1 Diabetes— died at the young age of 45. She received her medical degree from Dartmouth Medical School and was the Chief Resident in her Family Practice Residency at Brown University. Dr. Morisseau has been a faculty member for St. Mary’s Family Medicine Residency since 1995.
Dr. Morisseau specializes in the practice of obesity medicine and has been certified by the American Board of Obesity Medicine. She is dedicated to improving people’s lives with comprehensive treatment and evidence-based medicine. Dr. Morisseau places tremendous value in all clinical encounters and wants her patients to walk away from their office visit with a positive and enriching experience.
Dr. Morisseau spends her days off knitting, hiking, and cooking. She especially delights in cooking Sicilian broth on chilly Colorado days.

Michael Pramenko,

Dr. Michael Pramenko has been with Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, for over 23 years. He attended the University of Colorado for his undergraduate studies and received his medical degree at Dartmouth Medical School. Dr. Pramenko trained in Family Practice at the St. Mary’s Family Practice Residency Program.
Dr. Pramenko is passionate about public health. He has been heavily involved in the Mesa County Medical Society and Colorado State Medical Society, as well as the Physicians Health Care Congress, which works toward health care reform. Dr. Pramenko was named “Family Physician of the Year for Colorado” by the Colorado Academy of Family Physicians in 2007. Today, he serves as Chief Medical Officer for both Primary Care Partners and Monument Health, a clinically integrated health network.
Dr. Pramenko practices comprehensive family medicine, including obstetrics, pediatrics, preventive medicine, and geriatrics. He enjoys partnering with his patients to build long-standing relationships while assisting them to achieve their optimal health.
In his time away from the clinic, Dr. Pramenko spends time with his wife and children. He is passionate about improving conditions that impact the health of communities called the Social Determinants of Health (SDoH).

Lynn Price,

Dr. Lynn Price grew up in England. She received her Bachelor’s Degree in Life Science and her Master’s in Counseling from the University of Westminster in London, England. She taught biology, chemistry, physics, mathematics, and health. She was also a counselor in Suffolk, England, and a teacher in Meridian, Idaho before returning to school for her medical training. She received her medical degree from the University of Utah in Salt Lake City and completed the Family Medicine residency program at St. Mark’s Healthcare Foundation in Salt Lake City, Utah. Dr. Price joined Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, in 2017.
Dr. Price’s health care philosophy is rooted in whole-person care. She loves to work hand-in-hand with her patients, educating them about their decisions so that they can continue to live long and vibrant lives. Dr. Price finds her background in science and education extremely useful in achieving these goals.
When she’s not practicing medicine, Dr. Price spends time with her family and her dog. She enjoys long, rambling walks, listening to music (especially when her children are the musicians), and painting landscapes.

Dr. James Quackenbush is a Colorado native and has been with Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, since 1996. He is the past Medical Director for DOCS on Call, Primary Care Partners’ urgent care clinic. Dr. Quackenbush attended St. Mary’s Medical Center Family Practice Residency Program and received his medical degree from the University of Colorado Denver School of Medicine in 1993.
Dr. Quackenbush is passionate about urgent care medicine, sports medicine, motorsports medicine, and medical student and resident education. He enjoys coordinating the life-long health of all of his patients and is skilled in treating acute problems and managing chronic diseases.
In his spare time, Dr. Quackenbush can be found mountain biking, flying RC planes, and spending time in the sunshine with his wife and family.

Jennifer Roller,

Dr. Jennifer Roller was raised in Denver and has been on the Western Slope since 2007. She attended the University of Colorado for her medical degree and trained in family practice at St. Joseph Hospital in Denver. Before joining Family Physicians of Western Colorado (FPWC)—a division of Primary Care Partners— in 2014, she was a member of the faculty at St. Mary’s Medical Center Family Practice Residency and worked in family practice on Colorado’s front range for nine years.
Dr. Roller was drawn to family medicine because she loves all aspects of patient care; she especially appreciates the relationship formed with her patients when she is able to fully understand their circumstances. She believes the provider-patient relationship is formed when the practitioner truly knows the entire picture. Dr. Roller has clinical interests in women’s health, pediatrics, and adult medicine. She strives to deliver quality, personalized healthcare to all of her patients.
Dr. Roller enjoys reading, traveling, yoga, and all kinds of crafts when she is not at work.

Susan Sayers,

Dr. Susan Sayers has been with Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, since 1998. She grew up in Tucson, Arizona, and attended the University of Arizona for both her undergraduate and medical training. She completed her Family Practice Residency at St. Mary’s Medical Center.
Before pursuing medicine, Dr. Sayers worked as a research specialist at the Arizona Cancer Center and spent time in Thailand volunteering for the United States Peace Corps under the Malaria Control Program.
Dr. Sayers takes a science-based, patient-centered approach with her patients to help them achieve and maintain excellent health. Her clinical interests include diet, including whole food nutrition, prevention of disease, and healthy lifestyles. She loves to share how her own diet modifications to plant-based nutrition have helped her overall energy levels and health. She is a Food for Life Instructor working to empower people to take control of their health by changing the way they eat.
Dr. Sayers can be found enjoying our lovely community in her downtime, running, camping, or sailboating. She spends much of her spare time with family and is often reading or cooking.

Stephanie Shrago,

Dr. Stephanie Shrago grew up in Oklahoma City, Oklahoma. She attended the University of Colorado for her undergraduate and medical degrees and trained in Family Practice at St. Mary’s Medical Center. She has been with Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, since 2007.
Dr. Shrago has volunteered her time in community health clinics throughout Colorado serving homeless and underserved populations. She has also been the Reach Out & Read Coordinator for Grand Junction, focused on parental involvement in reading and early childhood literacy outreach. Dr. Shrago is excited to guide patients in reaching their optimal health goals. She embraces the changing world of medicine, where new and cutting-edge advances are constantly being made, and appreciates using the physician-patient relationship to aid the community in living their most fulfilling lives.
Dr. Shrago has an adventurous spirit and her activities outside of work include running, mountain biking, hiking, paddle boarding, traveling, and spending time with her family.

Paul Sturges,

Dr. Paul Sturges has long called Colorado home. He grew up in Colorado Springs, the son of a physician, and practiced medicine in California for fifteen years before returning to his home state. Dr. Sturges obtained his undergraduate degree from Union College in Lincoln, Nebraska, and his medical degree and Family Medicine training from Loma Linda University in California. In 2004, he received the prestigious “Award of Excellence in Innovation” from The Talbert Medical Group.
Dr. Sturges’ mission is to provide superb, compassionate care to all of his patients. He appreciates all aspects of family medicine, but he has a special interest in adult medicine. He emphasizes prevention to help patients avoid more serious issues down the road and prides himself in taking the time to listen to each patient and address their individual needs.
Dr. Sturges has a plethora of out-of-work interests, including videography, anything high-tech, and spending free time with his family. He loves the great outdoors and is often mountain biking, surfing, windsurfing, camping, snowboarding, and skiing in his time off.

Christine Welsh,

Dr. Christine Welsh grew up in Pennsylvania and received her medical training at Lake Erie College of Osteopathic Medicine in Erie, Pennsylvania. She continued her studies earning a Master of Science Degree in Medical Education and was the Chief Resident in her Family Practice training at Millcreek Community Hospital in Erie, Pennsylvania.
Dr. Welsh prides herself in taking a holistic approach to medicine. She is trained in medical acupuncture and osteopathic manipulation. She values the time that she spends with her patients and encourages each individual to take an active role in their health care in order to be their best, most healthy selves.
Living in Western Colorado, Dr. Welsh is extremely acquainted with all aspects of outdoor recreation. In her time off, she enjoys hiking, climbing, cycling, swimming, and can be found kicking around a ball in the yard with her kids. She also volunteers her time on mission trips, as a youth group leader, and for community health screenings and youth health education.

Peggy Wrich,

A Colorado native, Dr. Peggy Wrich grew up in Hotchkiss. She has been with Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, since 2011. In addition to practicing family medicine, Dr. Wrich also provides obstetrics, prenatal care, and delivery. Dr. Wrich received her Doctor of Osteopathic Medicine degree from Des Moines University and trained in Family Medicine at St. Mary’s Medical Center.
Dr. Wrich is privileged to support women through their pregnancy and postpartum journey and provides individualized, high-quality care throughout all phases of prenatal and postnatal care. Dr. Wrich also specializes in pediatric and adolescent medicine, and cares for the whole family, from newborns to adults.
Outside of work, Dr. Wrich spends time outdoors with her family exploring Colorado’s glorious mountains and rivers. She also volunteers in community schools educating our youth on health-related topics such as smoking and sex education.

Nancy Bauer,
Nurse Practitioner
Nancy Bauer is a Primary Care Nurse Practitioner from Wisconsin who has been with Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, since 2017. Before joining FPWC, Nancy worked at the Department of Veterans Affairs for almost a decade. She received her training as a nurse practitioner from the University of Wisconsin.
Nancy specializes in geriatric medicine and adult health. She is an expert in the study of aging and the aging process and focuses on treating the whole person, including medical, social, and emotional needs. She strives to ensure that all adults receive informed, cutting-edge care. Nancy believes that the doctor-patient relationship is a key to healthy aging, and looks forward to helping her patients work toward optimal health.

Mikayla Gase grew up in a small town near Akron/Canton Ohio. She completed her Bachelor of Science in Nursing (BSN) degree at the University of Akron in 2017. She then went on to complete her Master of Science Family Nurse Practitioner (FNP) degree from Chamberlain University in 2021.
Mikayla was an Emergency Room RN prior to becoming a Nurse Practitioner. She worked full time through the Covid-19 pandemic while going through grad school full time. She previously worked full time at Docs on Call prior to transferring to Family Physicians of Western Colorado. She has been with Primary Care Partners since 2021. Mikayla enjoys providing care for acute problems but has been broadening her knowledge with chronic disease management.
Mikayla visited Colorado for the first time in 2019 and knew this is where she wanted to live after grad school. She moved to Grand Junction, Colorado with her husband in Fall of 2021. When she’s not at work, Mikayla enjoys fantasy football, reading, exercising, snowboarding, playing with her three dogs, camping, hiking, exploring new places in Colorado, visiting national parks, and spending time with friends and family.

Jenna Berguin,
Nurse Practitioner
Jenna Berguin has been with Family Practice of Western Colorado (FPWC), a division of Primary Care Partners, since 2017. She was thrilled to return to her hometown of Grand Junction after working on the Front Range at the Caritas Clinic at Saint Joseph Hospital as a Nurse Practitioner and at Saint Joseph’s Hospital as a registered nurse. While at St. Joe’s, she received the “Good as Gold” Caregiver Award.
Since she was a young girl, Jenna has had the desire to be a nurse. Her skilled acumen and calm and compassionate bedside manner, make her an exceptional provider, which translates into a loyal patient panel and long-term relationships.
Jenna spends most of her time away from work with her young children or in Colorado’s great outdoors enjoying year-round activities.

James Haraway,
Nurse Practitioner
James Haraway has been a Nurse Practitioner on the Western Slope since 1998. He received his Master of Science in Nursing from Vanderbilt University and his bachelor’s degree in Nursing from the University of Alabama at Birmingham. James has been with Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners since 2019, and previously worked with Docs on Call, Primary Care Partners’ urgent care clinic.
James has experience in urgent and emergency care, surgical care, and family medicine. He treats patients of all ages with an emphasis on health promotion and disease prevention, focusing on exceptional service to his patient base.
James’ favorite board game is Ticket to Ride. You’ll find him hiking trails off the beaten path in his downtime.

Danielle Kerns,
Nurse Practitioner
Danielle Kerns is from an Air Force family and traveled a lot as a child. In 1996, her family relocated to Greeley, CO, and she has been a Colorado resident ever since. Danielle attended Colorado Christian University in Thornton, CO where she received her Bachelor in Nursing in 2014. She then went to complete her Masters in Nursing at Rural Family Nurse Practitioner Program from Regis University in Denver, CO in 2023.
Danielle Kerns is passionate about caring for people and is tenacious with staying up-to-date with healthcare education and developments to Family Physicians of Western Colorado. She is currently expanding her knowledge of hormone replacement. Her areas of interest include holistic care, women’s health, and wound care. She will be able to perform DOT exams at FPWC.
While living in Salida, CO, Danielle worked with the local family practice, high school, and Colorado Mountain College to develop CMC Internship Program for local high school students. The program helps students receive college credit and valuable experience to help guide their career path in the medical field.
When not in the office, Danielle enjoys spending time on the lake and loves to fish and hike.

Michelle McDermott,
Nurse Practitioner
Michelle McDermott grew up in Olathe, Colorado. When Michelle was in high school, she cared for her mother as she underwent a series of cancer surgeries and treatments. This personal experience fueled Michelle’s passion for medicine and her pursuit of a nursing degree from the University of Northern Colorado, straight out of high school.
Before earning her Doctor of Nursing Practice (DNP) from Colorado Mesa University, Michelle worked as a nurse at Grand Valley Surgical Center, Colorado West Otolaryngologists, and St. Mary’s Medical Center. She joined Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, in 2019.
Michelle maintains a humble appreciation for the complexities of family medicine. She strives to provide optimal patient care and enjoys treating patients from all walks of life.
Michelle loves the beauty of the Western Slope and can be found hiking, swimming, baking, and reading on her days off.

Lora Page,
Nurse Practitioner, Certified Diabetes Care & Education Specialist
Lora Page joined Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, in 2006. She was born and grew up in Grand Junction. Lora received her Bachelor’s degree in Nursing (BSN) from the University of Northern Colorado and a Master’s Degree in Nursing (MSN) from the University of Colorado.
Before joining Family Physicians of Western Colorado, Lora worked at St. Mary’s Medical Center as a Diabetes Nurse Educator and at Texas Children’s Hospital in Houston in the Diabetes and Endocrine Clinic. She is certified as a diabetes educator and a Family Health Practitioner. In 2020, Lora was a speaker on Diabetes Technology at the National Nurse Practitioner Conference. As a long-time member of the American Diabetes Association and past chairperson for America’s Walk for Diabetes, she is passionate about diabetes maintenance and education for members of the Grand Valley.
Lora enjoys spending time with her very large extended family, and also participates in her church choir.

Sarah Rooney,
Nurse Practitioner
Sarah Rooney grew up in the mountains in Wyoming. She received her Bachelors of Science in Nursing from the University of Wyoming and her Master of Science in Nursing from the University of South Carolina. Prior to earning her MSN, she worked in multiple rural hospitals where she gained experience in surgical, orthopedic, and acute care. Sarah joined Family Physicians of Western Colorado (FPWC), a division of Primary Care Partners, in 2022.
Sarah’s passion for healthcare started at a young age after seeing the positive effect that caring healthcare providers had on a family member suffering with a complicated illness. Keeping this in mind, Sarah works to create relationships with her patients. By collaborating with each of her patients, she can address and treat their health concerns on a more personalized level. Sarah is also passionate about preventative health and helping her patients to avoid serious complications later in life.
Sarah has been a resident of Grand Junction since 2021. She spends her free time, hiking, baking, and hanging out with her family.

Dr. Andrew Mohler is so caring and empathetic. Dr. Mohler has been my doctor for many years. As a matter of fact, I have been with this practice since 1987!! This practice has always been very professional when dealing with my entire family and any of our medical concerns.

From the moment the appointment was made I was very impressed by the professionalism of the Front Office! During my first visit, I was asked about my medical history, current medications, and the reason for my visit. The Nurse Practitioner who saw me was extremely competent and compassionate. Lastly, I am comfortable with my treatment plan. I felt very comfortable waiting in a clean and art-filled waiting room.
Every time I have been here, it has all been excellent from A to Z.
